Immunization Requirements for Middle SchoolFor school years 2012-13 and 2013-14 (NOTICE: The Denton County Health Dept. can no longer give immunizations to children who have private health insurance--they must get their vaccines from their primary doctor. Those children who have Medicaid can still get their vaccines from the Health Department.) |
8th Grade--no additional vaccines are needed. 8th Graders MUST have received the three vaccines listed below for 7th Grade. Information on the form will include the child’s name and date of birth; a list of required vaccines for which exemptions may be requested; a statement for the requesting parent/guardian to indicate their relationship to the child; and an acknowledgement that the parent/guardian has read the attached information, "The Benefits and Risks of Vaccinations". Parents/guardians will then have to sign the form in front of a notary public. Q. How many vaccine exemption affidavit forms can a parent or guardian request at one time? A.
