Immunization Requirements for Middle School

For school years 2012-13 and 2013-14
 
(NOTICE:  The Denton County Health Dept. can no longer give immunizations to children who have private health insurance--they must get their vaccines from their primary doctor. Those children who have Medicaid can still get their vaccines from the Health Department.)
 
 
 8th Grade--no additional vaccines are needed. 8th Graders MUST have received the three vaccines listed below for 7th Grade.
 
7th Grade:
In addition to the vaccines listed below (see 4th - 6th Grade list), 7th graders need the following three (3) vaccines:
1) Td booster
2) 2nd Varicella (chicken pox)
3) Meningitis
 
4th - 6th Grade:
4-5 doses of DTaP/DTP (4 doses meets the requirement if the 4th dose was received on or after the 4th birthday).
3-4 doses of Polio (3 doses meet the requirement if the 3rd dose was received on or after the 4th birthday)
2 doses of MMR
3 doses of Hepatitis B
1 dose of Varicella

Information for Parents who want to obtain a
Vaccine Exemption for Reasons of Conscience

Parents or guardians need to request a vaccine exemption affidavit form in writing or via a secure online request. Each child’s name and date of birth must be included in the request. Written requests must be submitted through the U.S. Postal Service, commercial carrier, fax, or online.

Phone and email requests cannot be accepted.

Households requesting an affidavit for more than one child should submit only one request form. The request should include the name and date of birth of each child living within that household who needs affidavit forms.

 
Q. What information will be listed on the vaccine exemption affidavit form I receive?
A. Information on the form will include the child’s name and date of birth; a list of required vaccines for which exemptions may be requested; a statement for the requesting parent/guardian to indicate their relationship to the child; and an acknowledgement that the parent/guardian has read the attached information, "The Benefits and Risks of Vaccinations". Parents/guardians will then have to sign the form in front of a notary public.
 
Q. How many vaccine exemption affidavit forms can a parent or guardian request at one time?

A. Parents/guardians can request up to five vaccine exemption affidavit forms per child.
