• 2017-18 ATHLETIC PHYSICALS AND RANK ONE ON-LINE PAPERWORK
    (due Wednesday, August 16, 2017) 
     
    7th, 9th or 11th grade athletes and/or athletes who plan to participate in athletics, as well as, those who are new to Denton ISD in the 2016-17 school year must have a physical.
     
    8th graders will not need a physical if and only if they had a physical during their 7th grade year.  If an incoming 8th grader was not in athletics during 7th grade, they will need a physical in order to participate.

    Any athlete planning on participating in sports, during his/her seventh grade, freshman or junior year will need a current athletic physical. This is a UIL rule, so the 2 year option does not apply.   You can get your physical from your family physician, but it MUST be on the current UIL physical form.

    5. Fill out all the red forms listed 
     
      Complete all seven forms.  On the medical history, please explain ANY 'yes' answer so we will be aware of your child's history and needs. The forms below need to be completed.
