Please note that both excused and unexcused absences count against a student qualifying for a VOE form.  This form is required for a driver's permit, license or license renewal.  A student must have at least 90% attendance in every class for the prior semester before a VOE form and be approved.  The current semester will be used if the student is already violating the 90% rule at the time of the request.

Students may pick up a VOE form in the Assistant Principal's office.  The form will be printed immediately.  We cannot fax, email or scan this form.  It must be picked up by the student.

Keep in mind that this form is only valid for 30 days from the date signed so be sure to have your student's driving test scheduled first.
