****Important Notification - Affordable Care Act (ACA)***
Denton ISD Risk Management was notified by the Internal Revenue Service (IRS) that several Affordable Care Act (ACA) Reporting Forms 1095-C were flagged due to an incorrect name, address, or social security number for the employee and/or dependents (spouse and/or children).
"Denton ISD will remain in compliance with all rules and laws set by the IRS-Affordable Care Act and will submit within 30 days all corrections electronically."
Upon receiving a request from Denton ISD Risk Management/ACA Specialist, please immediately upload the following information:
- Employees driver’s license
- Employee and/or Family Social security cards (see Laserfiche requested information)
- Fill out all requested information prior to submitting.
All required information should be uploaded through Laserfiche, should you have questions please contact:

Personal Information cannot be received over the phone or emailed for the security of your personal information. All information obtained is submitted to the IRS, and a new 1095C form will be printed and mailed to the employee for their records.
