How do I start a contract?
- From the Risk Management website: https://www.dentonisd.org/Page/88717
- Select the correct type of contract (Bounce House or All Other Events)
- Select DISD Booster Club & PTA Contract Agreement
- Select the download icon -->
- Save to desktop or file
- Exit out of Adobe and open saved file. Once opened all signature line and form is editable.
- Fill out form and click on the first signature line (document will be saved once signed).
- Email signed contract and any supporting documentation (invoice/agreement/series of emails) to campus Principal.
- Principal will sign and forward to Area Superintendent
- Area Superintendent will forward to Risk Management
- Risk Management will forward to Sherry Arrington to be returned to person listed as contact.

What do I need to provide for each contract?
Bounce Houses:
- DISD Agreement/contract Approval Worksheet
- Verify company appears on “Booster Club/PTA-Authorized Bounce House/Inflatable Vendors” Here
- Complete page two of the worksheet (verifying stickers and insurance appears on state websites)
- Verify and provide all backup documentation shows campus name and PTA or Booster club (example: Evers ES PTA or DHS Booster Club)
- All information has been sent to campus Principal
All Other Events:
- DISD Agreement/contract Approval Worksheet
- Verify and provide all backup documentation shows campus name and PTA or Booster club (example: Evers ES PTA or DHS Booster Club)
- All information has been sent to campus Principal

Does Booster Club/PTA need to provide a Certificate of Insurance (COI) to Risk Management
Campus PTA and Booster clubs need to provide a Certificate of Insurance (COI)at the beginning of each school year or as renewal of contract, that shows Denton ISD as the Certificate Holder as follows:

Does the vendor need to provide a Certificate of Insurance (COI) to Booster Club/PTA?
Campus PTA and Booster clubs should ask each vendor to provide a Certificate of Insurance (COI) for each contract that reflects the Campus name w/PTA or Booster Club as the certificate holders. This COI is not required to be attached to the contract.

Can PTA/Booster Clubs use any bounce house company?
- The only bounce houses that are allowed must be on the “Booster Club/PTA-Authorized Bounce House/Inflatable Vendors”
- If a new vendor wants to be used the Google form “PTA/Booster Clubs Application Request for Vendor” form must be completed and approved prior to be used on any contract.
- Allow 5 to 10 days for new vendor to be approved.
- Once approved vendor will appear on the “Booster Club/PTA-Authorized Bounce House/Inflatable Vendors”

As a PTA/Booster Club member completes a contract who should sign the contract?
