Special Education Records

  • This page is for Special Education records only.

    Requests for non-special education records (such as Section 504, RTI/MTSS, attendance, nurse, discipline, or email records) must be submitted through the campus.

    Parents/Guardians requesting copies of their child’s Special Education records should complete the Online Records Request Form.

    Requests from other districts should be submitted through TREx. If records cannot be transferred through TREx, please download, complete, and email the Records Request Form.

    If you are unable to complete or submit the form online, you may print and deliver it to the Special Education Records Office located in the Giese Student Services Building, or fax it to 940-369-4972.

    Once the records are ready, parents/guardians will be contacted and notified that the records are available for pickup.

    Please note that valid photo identification (such as a state-issued driver’s license or ID card) is required when picking up confidential Special Education records.
