      Teacher Time/Student Activity Form

      • Parents, please remember that it is your responsibility to submit this form to the ESD Main Office at least 48 hours before the activity starts. Please fill out the form completely and submit it to the ESD main office or give it to one of the ESD Instructors at your child's campus. It is vital that this form is completed and returned in order for your child/children to attend activities safelyWhen a child has an after-school activity, they must check in with their ESD Instructor before going to an activity in order to avoid being considered absentAfter you have filled this form out completely and the teacher has also signed it, please do one of the following:

        If the ESD Instructor does not have a Teacher Time/School Activity form on file, the child will not be allowed to attend the activity a second time. This includes but is not limited to:

        • Helping a teacher in the classroom 
        • Tutoring
        • Clubs (Any/All) 
        • Choir
        • Choir Programs 
        • Scouts (only if held on campus)
        • Guitar
        • Golf 
        • Safety Patrol 
        • Student Leadership
        • Wolves of Compassion 
        • Ballet
        • Christmas Performance 
        • Talent Show 

      Ongoing Absence from ESD Form

      • This is different from the activity form. If your child(ren) has a regularly scheduled event that will prevent them from checking in and/or attending ESD for a certain day each week or possibly a certain week each month please fill out this form. Giving us this form will keep you from having to call in an absence (please see our Non-Call-In policy in the Parent Manual on p. 301 & 400) only on the days listed on this form. This may include but is not limited to the following:

        • Walker for a certain day of each week
        • Bus rider for a certain day of each week
        • Car rider for a certain day of each week
        • Other parent’s week to pick up from school but not from ESD

      Notice of De-enrollment Form

      • Notice of de-enrollment can be submitted/emailed at any time, even if school is not in session. 
        If your child attends ESD for any number of days (even 1 day) in the month, the full tuition of $215 is due.


         This form is used to withdraw your child from ESD. After you have filled it out completely please do one of the following:

        • Print the form and bring it to the office at 1400 Malone St. Portable 46, Denton, TX 76201

      Transfer Request Form

      • If you are moving to another school zone and would like to keep your child in the ESD program, please fill out this form. After you have filled it out completely please do one of the following:

        When we receive the form, the ESD Program Director will review your request and let you know as soon as possible if there is availability at the new school.
